Registrations
To register with the practice please speak to one of our receptionist’s who will provide you with the relevant forms to complete.
Please bring with you 2 forms of identity, one photo id (driving licence or passport) and one proof of address (utility bill within the last 3 months)
Once the forms have been completed and the correct identification has been produced the registration will be processed but not complete until acceptance from the Health Authority has been received. If there is a problem with the registration a member of staff will contact you.
At the time of registration you will be asked to make an appointment for a New Patient Health Check at your convenience. This will be carried out by our Health Care Assistant or Practice Nurse and will give you the opportunity to discuss any existing health problems you may have or medications you currently take as well as recording height, weight etc.
There is no need to contact your previous GP as the Health Authority will recall your medical records and forward them to us.
